Job Description:
Responsible for all Service Coordination activities. Oversees the hiring, training and work of all Service Coordinators, Service Coordination Managers and ancillary staff. Plans, provides resources and directs activities for Service Coordination. Work scope is broad in nature, usually requiring considerable creativity, originality and ingenuity. Works with the Managers of Service Coordination to ensure the case management process of assessing, planning, implementation, coordination, monitoring, and evaluating services and outcomes is pursued to maximize the health of the Member.
